Posted on Mon, 5 Jan 2015
Question: My husband,77 years young, has myasthenia gravis and takes mestinon 4 times per day. Over the past year he has experienced clear watery substance flowing from his nose when he bends over. He has had no head trauma. But, the drug does cause excess mucus causing him to clear and spit. Should I be worried

Brief Answer:
Its nasal and sinus secretions

Detailed Answer:
Hi,
Thanks for asking.
I am Dr. Prakash HM and I will be answering your query.
Based on your query, my opinion is as follows:

1. Mestinon is known to cause increased secretions. In the nasal cavity, in the bronchial tree and in the sinus too.
2. Normally in standing position, all secretions from sinus and partly from nasal cavity gets drained into oral cavity.
3. When he bends, the secretions come outside nose. Not to worry.
4. If you are thinking of head injury and fracture skull, CSF drainage, this long would have led infection to travel to brain.
5. You can also do a simple test using a kerchief to collect secretions. If it becomes hard, its nasal or sinus secretions. If soft, its CSF from brain.

Hope it helps.
Any further queries, happy to help again.

When you do the handkerchief test ... Am I assuming you tissue paper as opposed to cloth, and when the liquid dries, the tissue is stiff.

Stiff tissue, nasal secretions

Tissue paper will also work as well as any cloth.
If its stiff, nasal secretions.
