What Causes Chest Pain, Bloating, Short Breath And Belching?

Question: Hello doctor, my name is XXXXXXX i m 32 years old male, 1.72m for 73kg. Since a few years, i ve been suffering from chest pains of the squeezing type in the epigastric region, typicaly at night ( they woke me up or cant fall asleep because of them) and in the early morning, i got a all package of other symptoms like short breath , bloating, lot of belching (sometimes forced to provide brief relief to the squeezing) and exercise intolerance, i get innapropriate sinus tachycardia that can last for many hours when i push to hard ( i use to do lot of sport, can t now). All those symptoms are getting worse and worse as time progress. I have had all the heart tests done Coronary CT scan , 48h holters, echo and stress test wich all came back ok , had lung tests too wich also came back ok. 9 months ago i had an endoscopy wich showed a small hiatus hernia and few small erosion from gastrisis inside stomach. Oesophagial tube was fine and no helicobacter or things of the sort. Over the years I ve tried a lot of medecine including PPI and went as far as seeing a psychiatrist and starting a therapy as well as taking a Benzodiazépine treatment as i was told many times by my GP that it might be an anxiety issue...unfortunetly nothing worked. I also did a all lot of lifestyle changes and i m verry carefull with my diet but still no signs of improvement . My question are: Could a small hiatus hernia could produce all those symptoms?? How likely it is to get misdiagnose during endoscopy and not notice the real size of the hernia? What are my options at this point, surgery? XXXXXXX t tried chiropractor , any thoughts on that? I apologize to make a post this long and ask soo many questions, but even thought i m sufering and quiet desperate at times, i want to get to the bottom of this and fix the problem. Brief Answer: Following suggestions.. Detailed Answer: Hi, I have gone through your medical history and understood your concerns. Your clinical symptoms are indicative for hiatal hernia and GERD. With regards to the size, I suggest to run : - Barium swallow study to evaluate the size better - pH study for GERD - abdominal ultrasound to rule out gallbladder I advise to: - continue PPIs medications - start domperidone - take 1 more pillow to sleep at night - do not lay down immediately after eating Hope it was of help! Thank you for answering my queru.Two last question, does small sliding hernia run the risk to get strangulated? I asked because the squeezing pain i feel in the epigastric region feel like something is "strangulated" and lastly i m loosing a lot of weight at the moment , much more than usually for the same diet, is this typical for hiatal hernia?

Brief Answer: Your clinical symptoms not same to strangulation. Detailed Answer: Hi XXXXXXX Sorry for the inconvenience caused. My system has run several problems recently. I am grateful for your patience. With regards to your current problems, a small sliding hernia does not run the risk to get strangulated. Instead, big hernia are more at risk. There are many of my patients being diagnosed with small sliding hernia and lived longer with no strangulation problems. I cannot say the same for other my patients with hernia bigger in size, they had gone several severer problems from strangulations. Furthermore, your clinical symptoms are not similar to strangulation. On contrary, strangulated hernia is an acute problems and not similar to yours. Losing weight is not related to hiatal hernia. It might be related to : - diet you are following (to get symptoms improved) - or because you have been eating small portions of foods - or another cause could be more physical activity.
