Question: Hi I had asked you the questions on my MRI reports about ankle and knee and seeing that you are radiologist is it quite common to see edema without injury or fluid in joints ?

1. Research studies have been done to know the presence of fluid in ankle joint in normal people and those having an injury or condition involving ankle joints.

2. Results show that the amount of fluid normally present in joints varies from
0.13 to 3.5 ml. The presence of joint fluid in ankle joint is acceptable in normal joints even without injury.

3. The fluid volume imaged by MRI in patients with the usual foot disorders does not seem to differ from that seen in persons with normal ankles.

4. Regarding the presence of edema in the calcaneum or heel bone, literature says that calcaneal bone marrow edema is found due to traumatic and non traumatic causes. Among traumatic causes, it might be seen in stress and occult fractures and ligamentous avulsions. Injury or inflammation of Achilles tendon and peroneus longus tendon abnormalities can also produce isolated calcaneal bone marrow edema.

Other non traumatic causes of calcanuem bone marrow edema includes, plantar fasciitis, osteoarthritis, inflammatory arthritides, and impingement.

Ok very informative, however I did forget to put down the edema was in the subcutanous fat around the weight bearing area of the cancanuem or heel the report says.

1. Subcutaneous edema at the heel region is usually seen in a condition called plantar fasciitis. This is a painful condotion.

2. There is also minimal subcutaneous edema around the calcaneum which might appear without any symptoms and has no association with plantar fasciitis. This is more of an incidental finding. It is possible that this is the explanation for your MRI ankle report finding.
