What Causes Buzzing Sound In Ear After Tympanoplasty?

Question: Hello Doctor,

I had an injury on my left ear on April 2013 and got Tympanoplasty done on december 2013.

All is fine now....but i still have buzzing sound constantly 24 hours.

My doctor advised me to take stablanz pv for a month .Am on medication now since 1 week.i have to see him again after a month.

My question to you is:

1) will this buzzing sound in my ear will ever go or will remain permanently.
2) even after taking medications if the buzzing sound donot go then does it mean all my life i will have this or r there any more procedures to tackle this.

3) as my doctor advised me to take stablanz pv in the night ...i usually miss due to my social life..

is it ok to take it in the morning?

1. The constant buzzing sound is known as Tinnitus. Was the Tinnitus present since the ear injury or did you become aware of it after the ear surgery? Is the Tinnitus on one side or both? If it is in both, then it was most likely due to the ear trauma (depends on the details).

2. Get your ears examined to rule out any ear drum perforation or wax build-up which are common causes for Tinnitus. Insist on seeing your eardrums with an endoscopic camera on a video monitor.

3. Check your Tympanoplasty operative notes to check whether any cartilage was used in the reconstruction. The slight added weight or pressure on the inner ear may be causing the Tinnitus.

4. Sometimes this Tinnitus is unexplained. It may be due to handling of the delicate middle ear structures or you may have become aware of it after the surgery due to some residual hearing loss. Decreased hearing also makes one more aware of tinnitus. Get a PTA (Pure Tone Audiogram) done. If you have a hearing loss, amplification may help mask the tinnitus. If your eardrums are intact, you may also get your middle ear pressures checked (Impedance Audiometry or Tympanometry).

5. Many normal hearing people also experience Tinntus. Hence it is not possible for anyone to predict the course of this Tinniitus. However, with treatment it may be controlled in half the treated patients.

6. You are being given Tab. Stablanz PV which contains betahistine and gingko biloba. Gingko biloba is commonly used as a first line treatment for Tinnitus. You may take it in the morning.

7. Caroverine infusion followed by capsules for two months is a new treatment. Other treatments such as transtympanic steroids, HyperBaric Oxygen Therapy (HBOT), multi-vitamins and anti-oxidants may also help in reducing Tinnitus.

8. Kindly let me know the results of your investigations. This will help in suggesting further treatment.

The Tinnitus started after the surgery.The tympanoplasty was done due to ear damage due to trauma while playing.

Only on my left ear.

is there any way that Tinnitus will remain all my life?

few more questions:

can i hit the gym?
can i swim with head cap.

any diet plan ,usually i drink 3 to 4 days a week.180ml /per day.

any problem with this?

1. Theoretically, there is no way to prove trace back whether the injury, surgery or some other process caused the Tinnitus. Practically, no revision ear surgery can guarantee any reversal.

2. All the above activities are safe and will not affect the Tinnitus.

3. You may try each of the treatments mentioned above to try to decrease the intensity of the Tinnitus.
