What Causes Burning Sensation Inside The Tip Of Penis?

Question: Dear Doctor,
I am 42 year old healthy male. I have been having slight burning sensation inside the tip of my penis(towards the bottom 2 cm inside from tip) when I start passing urine, for last 3 to 4 months. There is no urinari tract infection and ultrasound of main organs in the abdomen is normal. Simultaneously, I have started having a slight sensation in my anus for last 1 month or so, which is now turning into slight pain occasionally as I try and squeeze the end of my anus. The feeling is similar to what one feels, after you squeeze your anus post constipation. I have also been on Doxycyline & Lactic Acid Bacillus Capsules twice a day for last 10 days without any improvement. All the above feelings of burning in penis and anus is not continous. I have no discomfort most of the day. Slight burining sensation/slight pain in penis is only during the beginning of unrination and reduces as I pass urine. Please advise if there is cause of worry & help. Additional Information- 1.I have been under lot of stress as my son is terminally ill cancer patient and I have family problems. 2. I have a strong sexual urge/appetite & like to have sex regularly, which I have not been able to fulfill regularly because of various problems.

Thanks for your query,based on the facts that you have posted it appears that you have burning while passing urine and pain around anal region.This could be likely due to infection of prostate .or Chronic fissure in anus.

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Are you taking any medications for your psychological problems ?
2) Do you have discomfort in your testicles?
3) Do you have fever ?
4) Do you have painful ejaculation?
5) Do you have constipation ?
6) Do you have pain in anus before passing stools ?

Please consult qualified Urologist for clinical and digital rectal examination and get following basic tests done to confirm the diagnosis.
1) Semen test and culture done to find out the organisms causing this infection and antibiotics to which they are sensitive to.
2) Trans rectal Ultrasound Scanning (TRUS)

Please take broad spectrum antibiotics like Cefexine along with urinary antiseptic like Nitrofurantoin twice daily and urine alkaliser thrice daily.Later on switch on to appropriate antibiotics as per culture report.
Get the prescription of medicines from your family Physician.

Ensure to drink more water.To keep your urine dilute This will help to control dysuria.

Further treatment will depend upon the results of these tests and final diagnosis.

Could be due to inflammed urethra.

Thanks for feedback.
Pain in anus is likely to be due to chronic fissure in anus and pain in penis is refereed pain due to same nerve innervation of penis and anal canal by Pudendal nerve .

Please consult qualified General surgeon for rectal and proctoscopic examination to rule out the same .

Take mild laxatives like Cremafin and pain killers like Diclofenac twice daily for a week and observe for any improvement.
