What Causes Breathlessness On Little Exertion In An HIV Patient?

Question: Dear Sir,

My Father is 61 years old and his medical History is as below:

2007 - He has undergone a bypass Surgery in Bhaillal Amin Hospital, Baroda
2011 - Detected with HIV. Undergoing regular treatment since then with Dr. XXXXXXX Parmar, XXXXXXX Current CD4 Count is in range of 300-350

Apart from these he was addicted to Smoking and drinking and due to these His lungs are functioning at 30%-40% efficiency since last 5 years. He was admitted to Hospitals almost every year since last 5 years due to Cough in Chest, Deficiency of O2 due to problem in lungs and Asthma.

Since last 10 days he has come to XXXXXXX for short period of time to stay with me. However his health has deteriorated to some extent and he is facing breathing issue even if he is walking for few meters.

I would Like to know which specialty doctor to consult (name of Dr. in particular if possible) so that I can get proper medication for him considering his Medical history as mentioned above. He has now given up smoking completely since last 1.5 year. however He takes drinks occasionally.

Your father is having breathlessness on little exertion (walking few meters).
He is also an having respiratory illness in the past. He also had CABG (Coronary artery bypass grafting).
So we should rule out lung and heart related diseases.
So consult pulmonologist and get done clinical examination of respiratory system and PFT (pulmonary function test) to rule out acute worsening of bronchitis or asthma.
Also consult cardiologist and get done ecg, 2d echo to rule out early heart failure.
He might need long acting inhaled bronchodilator (formoterol or salmeterol) and inhaled corticosteroids (ICS) (budesonide or fluticasone) if it is respiratory problem.
He might need diuretics and other cardiac supportive drugs if it is cardiac issue.
I don't know the names of doctor in XXXXXXX but you'll should consult multispeciality hospital where pulmonologist and cardiologist are available. XXXXXXX Asia hospital is good option in XXXXXXX So take your father in this hospital.
