Brief Answer:
you need to get examined thoroughly to know the cause at first..

Detailed Answer:
Hello XXXXXXX Welcome to HCM..

I've gone through your query and understand your concern..

Yes I've reviewed the PFT report in the other query which is already addressed by the fellow Pulomonologist..

So let me assure you at first that since the PFT is normal , so your lung functional part is just fine, it's good that you've quit smoking , so congratulation for eliminating one of the most common culprit..

At this age, with this type of history you should be investigated in details to rule out any associated/underlying cardiological disorder and/ even allergic disorder(which the pulmonologist has also pointed out..), so I'm not going to ask you the same questions repeatedly, in fact I'll answer in light of all the details gathered so far from both the queries..

Coming to your questions :

1) Breathing trouble can very well be allergic response to dry cold air of air-conditioning , dust/mold or may be underlying cardiac anomaly- needs further investigation as well as physical examination by a Cardiologist (7 years a quite long duration, so you need to undergo for a fresh 12 lead ECG, 2D-Echo, CBC with absolute eosinophil count, Chest X-ray(PA view), Renal function test with electrolytes)..

Since you're already on Montelukast so I'd advice to start taking Tab LEVOCETIRIZINE 10mg once daily at bedtime along with..

2) With such a chronic past history of smoking, it might take several weeks to months for the respiratory tract and lungs to heal, so please be patient and start following all the advises strictly..

3) Don't worry to much regarding BP, even though your on Telmisartan , it's mostly under control, but target BP should be kept under 130/80 mmhg, however it's a variable parameter which can be increased due to any kind stress(yes, breathing difficulty is one of them)..

4) Complete relief can take time, from months to even year, depending on the current physical findings as well as the over-all immunity and body metabolism as well as discipline..

Avoid too much spicy foods/junk foods/dairy items/fizzy carbonated beverages as they tend to increase secretions as well as make persons more prone to acid-peptic diseases , in turn increases the respiratory problems also..also avoid any extra salt in diet..

5) To increase Lung capacity, try Deep breathing Exercises along with breath holding(Inhale -hold - exhale ) exercises(you can simply get in tough with your physician or any qualified physiotherapist for training)..

Take steam inhalation daily three to four times..stay away from dust and direct exposure of air-conditioning..

..Swimming can be tried for betterment (but need clearance from your Cardiologist at first after a full clinical examination),

6) Follow all the advises at first to find out the exact cause/cumulative causes for it, then the treatment will become easier and more specific..

7) Reviewed, present lung function is good, however for detailed evaluation, Chest X-ray and clinical examination is must..

Let me know if anything's not clear, I'll be glad to guide you..
