Brief Answer:
Hello dear. Ultrasonography is also required

Detailed Answer:
Hello dear. I have gone through the details. So a fross hematuria, with clots seemsmore suspicious for tumor bleed or stones rather than infection.
An ultrasound is warranted and if it is negative, then mri or cystoscopy must be planned.
Tumors of kidney and bladder behave in such fashion.
If it is pure infection, it should resolve in another couple of days. Continue with antibiotics and plenty of fluids.
Please revert back with reports.

Thanks and regards
