Question: My 23 year old son has bright red bleeding after a bowel movement. He has gone back in the bathroom twice since going and still has blood on the toilet paper.

Hi.
Thanks for your query.
Noted the history about your 23 year old son and understood the concerns.
Red bleeding after bowel movement in an young person is usually due to either piles or fissure.
The bleeding with Hemorrhoids/piles is painless, can be with or after stool, in drops or can be alarming sometimes. There may be prolapsing piles which can be felt.
The bleeding with anal fissure is always with pain, comes as streaks on stool or can be few drops after passing stool, never alarming, can smear on toilet paper.
Other rare causes are ruled out on examination only.

This can be diagnosed by:
Careful history.
Actual examination, per-rectal digital examination and ano-proctoscopy, done preferably by a General Surgeon.

Till then, I would advise him the following:
- Sitz bath in a tub with warm water.
- To calm down as this increases the stress and anxiety.
- Take plenty of oral fluids and normal full diet, having more fibers.
- Not to strain at stool.
- If there is attendant loose stool or any other abdominal complaints, please let me know.
