What Causes Blackish Lump On The Ankle While On Warfarin?

Question: My 86 year old father has a hard lump that has turned black and the blackness is spreading near his ankle bone. I told him he may have hit it and perhaps it was a hemtoma. He has kept it elevated for four days with no reduction of the blackness. He has kept ice on it and when he takes the ice off there is great pain. I am not sure if it qualifies as an emergency. He takes Warfarin.

Based on your query, my opinion is as follows:

1. Hematoma in patients on warfarin is a serious issue. Warfarin dose might require adjustment, if the hematoma has formed without any injury.
2. As it is painful, either the swelling is big and pressing on nerve or muscle and causing damage. Hematoma swelling needs to be drained or removed. It would not reduce with limb elevation as in case of hematoma, blood has come outside blood vessel.
3. An X-ray along with FNAC of the lesion is necessary. Meet your doctor to make sure there is no underlying tissue damage. If the swelling size is large, then it is serious issue.

No need to apply ice pack. If pain is severe, he will need to go to ER. If not, he can wait till tuesday. Any additional symptoms like breathlessness or weakness of arms or legs, then he will need to go to ER. Until then no problems for now.
