default
Posted on Tue, 5 May 2015
Question: Just a quick question,

Is prostate enlargemnt common under 38 years of age?
doctor
Answered by Dr. Panagiotis Zografakis (18 minutes later)
Brief Answer:
not so common but not impossible (8%)

Detailed Answer:
Hello,

benign prostatic hypertrophy occurs in 8% of patients aged between 31 and 40 years of age, so this is not normally expected in a 38 year old man.

Kind Regards!

Answered by Dr. Panagiotis Zografakis (7 minutes later)
Brief Answer:
I suggest an ultrasound scan

Detailed Answer:
If your urinalysis is normal (no blood, no sign of infection) and urethritis has been excluded then you should definitely look at your prostate. You haven't mentioned other symptoms besides feeling the need to urinate, so I suppose there aren't any.

An ultrasound scan of your urinary tract (including the prostate) may reveal the cause. If nothing can be found the you should contact a urologist.

Could this be cancer? In your opinion?

Or what else?
doctor
Answered by Dr. Panagiotis Zografakis (9 minutes later)
Brief Answer:
It could

Detailed Answer:
Cancer is a potential cause but urgency is not expected as the presenting symptom. Bleeding is the most common presenting symptom. The urinalysis should have detected microscopic bleeding, although this is sometimes intermittent.

Other potential cause may include an uncontrolled diabetes (high blood sugar causes increased urine production), overactive bladder (this is a neurological problem of the bladder - the urologist can diagnose it) and anxiety. Has urethritis been excluded by your doctor?

Other causes may apply in your case but they are less common.
Visiting the urologist would be your best next step.

Not what i wanted to hear as i was told by myndoctor that prostate cancer is very unlikely.

Can drinking alot of fluids be a cause?
doctor
Answered by Dr. Panagiotis Zografakis (5 minutes later)
Brief Answer:
Cancer IS unlikely

Detailed Answer:
Perhaps I should rephrase my previous answer because obviously you got the wrong message. Cancer IS NOT a real concern in your case but investigation is needed. Drinking a lot of fluids (including beers and coffee) may produce a lot of urine but you should be relieved every time you go to the WC. If your symptom involves a high volume of urine but no urgency then uncontrolled diabetes, anxiety and high fluid intake (or caffeinated drinks) can be the cause.

I hope this is clear now!

I don't have a high urgency. just a feeling to go. but when I go it feels that I have emptied properly and then feel I need to go a little while later. my diabetes is controlled well.
doctor
Answered by Dr. Panagiotis Zografakis (4 minutes later)
Brief Answer:
What about urine volume?

Detailed Answer:
You haven't commented on your urine volume. Do you have a lot of urine every time you empty your bladder or is it just a few drops? Do you drink caffeine drinks like coffee, cola etc? Do you drink beer? Are you suffering from anxiety?

Its normal urine. Plenty i would guess. Not drop.
doctor
Answered by Dr. Panagiotis Zografakis (4 minutes later)
Brief Answer:
This sounds like anxiety or too many liquids

Detailed Answer:
Anxiety and excessive drinking may cause that. You haven't mentioned your drinking habits regarding beer, coffee and other caffeinated beverages but if you do drink a lot of them (and water of course) you are going to have a high urine volume and frequent urination. Reducing the intake should reduce the volume.

Oh ok. It just seems I just can't empty fully even after having a good flow. maybe I am thinking too much? ??
doctor
Answered by Dr. Panagiotis Zografakis (2 minutes later)
Brief Answer:
Too much thinking = anxiety!

Detailed Answer:
Yes, it seems you do! This is called anxiety. But you should proceed with the investigation though. Anxiety is a diagnosis of exclusion!

what explains the weird feeling at tip of penis. I keep touching my penis. ? is this infection?
doctor
Answered by Dr. Panagiotis Zografakis (8 minutes later)
Brief Answer:
You said that no infection was found!

Detailed Answer:
If you just touch your penis then this is a symptom of anxiety too! If you have other symptoms like burning pain on the tip then urethritis is possible. Cystitis could cause similar symptoms, urgency prevails though.

I thought you've done tests and excluded infection! Is that true or not?

Urine test has been done and was negative. Would uretitis come up on the dipstick?
doctor
Answered by Dr. Panagiotis Zografakis (6 minutes later)
Brief Answer:
not always

Detailed Answer:
The urinalysis might be negative sometimes. A first voided urine should be examined for better results. Urethritis is usually accompanied by discharge that's evident in the first urination of the day (applying pressure on the tip of the penis would cause purulent discharge to come out).

Your symptoms do not suggest urethritis but I can't exclude it.

Ok thanks.

So with the symtomps that i do have.

In your expert opinion what do you think is possibly wrong?

Have had 2 urine tests with 48 hours and both negative.

Also bloods are normal.

Sorry this will be my last question.

You have been great!
doctor
Answered by Dr. Panagiotis Zografakis (4 minutes later)
Brief Answer:
Anxiety probably

Detailed Answer:
I would place my bet on anxiety! I would still ask for that ultrasound scan though...

Best wishes!
