Posted on Fri, 20 Jun 2014
Question: I have had itching and burning inside the vagina and the clit and I had white chunky discharge now starting on the anus. It is slowly getting better but it's starting on my anus now. I am sexually active but never had unprotected sex. I used monistat 1 and it made it even worse! Some days it's fine and other days it's worse. What can I do?

Brief Answer:
Yeast Infection

Detailed Answer:
Hello and welcome to healthcaremagic

I am Dr. Kakkar. I have gone through your concern and i have understood it.

I think yeast infection is the underlying problem for the symptoms that you have been experiencing. I would suggest an oral antifungal (Fluconazole 150 mg single dose) as a substitute to the local treatment.

The most common symptom of a yeast infection is itchiness in and around the vagina.

Other symptoms include:

-Burning, redness, and swelling of the vagina and the vulva
-Pain/burning when passing urine
-Pain during sex
-Soreness
-A thick, white vaginal discharge that looks like cottage cheese
-A rash on the vagina

You may only have a few of these symptoms. They may be mild or severe.

If I was the treating doctor I would have asked you to take a single stat dose of Oral Fluconazole 150mg.

You could also apply a topical antifungal cream, twice daily e.g clotrimazole cream is available OTC OR can also use a clotrimazole Vaginal Pessary once daily at night for 7 days.

An OTC antihistamine e.g cetrizine 10mg once daily for symptomatic relief from itching

Do you think it has anything to do with a STD?

Brief Answer:
Oral antifungal

Detailed Answer:
Hi.

The symptoms you experience are more suggestive of a candidal infection: White curdy discharge, itching and burning.

Though candida can be acquired sexually but it can happen otherwise as well like due to repeated vaginal douching, on oral antibiotics etc

I would suggest that you take an oral antifungal for a more complete cure.

I hope there are no erosions/ulcers/breaks in the skin of the vagina and perianal area? You can upload me a picture for confirmation.

Herpes is the only thing that I would like to rule out over here, specially since the anal area is also involved. However, the clinical picture of herpes is sufficiently characteristic to be diagnostic. It usually presents as grouped discrete erosions. The erosions are associated with symptoms of burning and pain. If this is not the case then most likely it is due to yeast infection and you would be better served with an oral antifungal.

Hi.

Yeast Infection can extend down to the anus, from vagina.

An oral antifungal is worth trying. Along with that you can also apply clotrimazole cream, twice daily.

Hi

Vulvovaginal candidiasis may recur just before each menstrual cycle (cyclic vulvovaginitis).

Symptoms may sometimes be aggravated by sexual intercourse.
