Brief Answer:
Seems like subcuticular infection

Detailed Answer:
Hi,
Thanks for your query, albeit short and specific.
Noted the history and understood the concerns.

The picture is quite clear and shows pustulous formation in a patch in the perianal skin - subcuticular infection thus causing soreness and pain.

There is also a skin tag that looks edematous.

I hope this answers your query.
Please feel free to ask for further queries.
Regards

Answered by Dr. T Chandrakant 17 minutes later
Brief Answer:
As detailed below.

Detailed Answer:
Thanks for following up,

Take a Sitz bath in a tub with warm water for soothing effect.
Take a stool softener or a laxative to keep stools soft for easy evacuation.
Take an anti-inflammatory like Ibuprofen if it suits you.
Apply liquid povidone iodine locally.
For proper antibiotic, you need a valid prescription from your Doctor.
All these will help you to get a relief and possible cure.

Once the pain and swelling go away a proper per-digital examination and ano-proctoscopy should help to get any primary cause and get treated accordingly.

I hope this answer helps you.
Please give feedback and ask for relevant queries if you feel that there is a gap of communication.
