What Causes Abdominal Cramps, Vomiting And Diarrhoea?

Question: I have a long history of IBS, and have had diverticulitis attacks a couple of times, both more than 10 years ago. I have an ongoing problem with constipation; Over the last couple of months, I have been pooping like a rodent - a few small acorn-sized balls every time I pee, but no large, satisfying poops. I knew i wasn't emptying sufficiently, so have tried eating more fiber, tried metamucil, etc all to no avail. And then yesterday, I had a gastric meltdown - very painful cramps, vomiting, diarrhea. Here's my question: Is it possible that poop just piled up inside me until my gut was so overloaded that it reacted? And - if that's the case - I'm thinking it might help me to just clean out every few months with polyethylene glycol. Is that a reasonable way to address what seems to be a recurring scenario for me?

Hello! Thanks for putting your query in HCM. I am a Gastroenterologist.

Abdominal cramps, with vomiting and diarrhoea suggest a GI infection for which I will suggest you to take plenty of fluids and electral powder to prevent fluid and electrolyte insufficiency. Also, take a course of probiotics along with mild antispasmodic like drotavarine. Meanwhile get your blood and stool examined (Complete haemogram and stool routine microscopy) to see for any infection and if infection is present, then a course of antibiotic like ofloxacin will also be required.

For constipation I will suggest, once this episode is over to take lots of fibres in form of fruits and vegetables and add psyllium fibre upto 20gm/ day. Also add polyethylene glycol 4tsf at night time daily. Titrate dose to make 2-3 stools per day. Take plenty of fluids.
