Question: i am 24 year old female. i have lower abdomen, pelvic pain and lower back pain which sometimes goes till my legs and sometimes stabbing pain near ovaries from past 3 months( since XXXXXXX 14 2014). most of the times it is mild but sometime increases (yet managable but i get scared).sometimes white discharge. I am seeing a gynae and she suspects infection but not sure. she prescribed doxy last month for 5 days but it didn effect my pain ,only discharge stopped which again came back later. Doc is concerned about infection coming back again and again and pain. she ordered ct scan and blood test which i am yet to have. i have done many ultrasounds which suggested pcod( i have it since 2 years ) , so only she ordered ct scan. i had done blood test (dec 13) which showed 4.94 rbc and 9.7 wbc .
i am on femilon from december for irregular periods becoz of which i have painful breasts and nausea(sometimes). could this pain be due to femilon?..Doc has also ordered for placentrax injection for ten alternative days and this is major concern as i got many negative reviews about this injection and checked on internet which shows its banned in many countries and suspected of spreading hiv kind of infections. I am really scared to take this injections and dont want to take it but my doc wants me to take this for pid. i have pcod from 2 years and had an abortion last year. please reply in detail and help.

Hi XXXX,

I went through your complaints with diligence. According to your symptoms, you need to be thoroughly investigated for pain.

I would first like to exclude urine infection as the cause of pain here. Hence I will first order urine routine examination.

Before giving my approving for CT scan, I would like to review the ultrasound report. If possible can you upload the report on this site using upload report option provided on this page. I need to note for specific points such as fluid in POD,any ovarian involvement or anything else in scan of whole abdomen.

You should also discuss about recurring/remaining pelvic infections with the treating doctor. If the infection is still present, then I usually prescribe doxycycline along with Metrogyl for 14 days along with some anti inflammatory drugs and also treat partner to avoid recurrent infection. You may also use vaginal tablets to clear discharge and vaginal infection.

Tab. femilon and PCOD is not the cause of pain. yes, femilon can cause nausea and breast tenderness.

Regarding Inj. Placentrex, it is individual choice for this treatment. And yes there are contradictory reviews. But this will not transmit HIV to you like this through inj. Rest assured and it will not cause any harm to you.

One more thing I will like to know about you that did have any surgery of any kind earlier.

Thanks for the reports; I have gone through them.

The ultrasound reports you have sent are both good and showing no evidence of PID. Also you have no surgery done earlier and abortion done 1 year back is also not the cause for pain. You may take Inj. Placentrex, no harm as advised by your dr.

For knowing the cause for pain, you can first get an ultrasound whole abdomen done, as the previous reports are focusing pelvis only, and if still no clue then proceed for CT Scan.

Hello doctor ,
I did urine test ( both culture and routine ) , blood test ( CBC with ESR ) , mauntox test , CT scan of abdomen and pelvis with contrast . I have uploaded the reports .

I showed to my gynaecologist yesterday and she said everything is normal . As I have been going to her she said now she ll not give me any more medication and gave 3 tablets , vitaminD , one for general well being ( forgotten the name , will let u know in next post ) and zenteel for worms .

She thinks I am thinking alot and everything is fine .

I told her that I m still having pain in my lower abdomen , back and lower back , pelvis ( front and back ) and thighs as well as legs . Its all over , sometimes increases in left side and sometimes right side , somtimes lower back , or stabbing pain in left middle or right abdomen or cramping type pain .Sometimes after urination I have burning sensation ( sometimes ) and urgency to urinate frequently but nothing comes out after going ( this also happens sometimes and not always ).

She said she has done all the tests and it seems nothin as it would have shown in the reports . She was concerned for infection like pid and abdominal tuberculosis but reports are clear . As my symptoms are very vague she thinks its only stress or something .

I pointed out in ct scan report its written subcentimeter reactive lymph nodes in small bowel mesentery but she said its very common and nothing to worry about .

Urine culture report also has something which I didn understand and doc was ok with it .

Blood test shows high rbc which was 4.5 last year . In nov 2013 it was 4.94 and now in april 2014 its 5.01. I donno y its goin on increasing . Is there something to worry about ? . I told my doc but she said its ok .
MCV is low , absolute lymphocyte count is raised and basophils are 0 . Please let me know if it's ok or no .

Last year my esr was 26 and now its only 2 !

Mauntox test is also cause of concern for me as I feel in 48 hours it was around 2 cm and he had called me before 72 hours n den it was less so he wrote it as negative . I told him it was more yest in 48 hour but he was ok with it . Does it increase n decrease or it shud be done in 48th hour or 72 hour ? . Nobody answered this question that y was it more yest and today less .

I am worried for any cancer , abdominal tuberculosis or infection but as ct scan is clear shud I go for further testing or no ? As more medications and scanning is also not good.

Will any cancer of abdomen n pelvis or tb or infection will be shown In ct scan ?

As I still have pain I m worried what and where to go or do next

Could this be related to rectum or colon infection or cancer ? Or this will also be shown in ct scan if it was present .

Please answer .

Hi XXXX,
I can understand your concern for pain. Your investigations are all normal and nothing to worry so far the reports are concerned.But the thing is you are not symptom free. The reactive subcentimeter lymph nodes in small bowel mesentry can be due to infection and nothing serious,as your dr. has rightly said.
The things not shown in CT scan cannot be commented upon. I don't think you need further investigation like colonoscopy or sigmoidoscopy to rule out colon or rectal disease. Take anti inflammatory and anti spasmodic drugs for some time for pain releif in addition to drugs given to you. If still no releif in pain, consult a surgeon then who can do a per rectum examination also and assure you regarding such diseases.
Regarding Mantoux test, the reading is taken between 48-72 hours. The patient is called after 48 hours and reading taken and it is final Less than 10mm is normal range.yours must be 2mm and not 2cm as it is written negative report.Sometimes, when we are in doubt and induration( redness seen on arm) is more than 10mm, we call the patient again after 72 hours total , or sometimes patient does not report after 48 hours and then we read the report after 72 hrs. and mention this also. Ideally,report given after 48 hrs. only.
Do not worry about ESR, CBC,They are all good.
Also don't be so negative, although we should be aware of all the things and keep ourselves updated . If any evidence of cancer, abdominal TB, some supportive evidence should have been shown in CT.

so if i had any problem with colon or rectal it would have shown up in ct scan?
as it was full abdomen and pelvis ct scan?

Could this be related to weakened bones ?

Mauntox test after 48 hours was checked by me only with the help of scale and it was around 2 cm( not sure how far i am right with the method) but next day it was much lesser and he wrote it as negative(around 70th hour).

so any big issue can be ruled out as ct scan is clear?

Hi.
As I told you that to have any problem with colon or rectum you need to consult a surgeon and get P/R examination done before going in for its detailed investigations.
The pain due to weakened bones should be in pelvis , I mean difficulty in walking or joint pains and not pain abdomen also.

I don't think you have read the Mantoux report correctly. You must have read the redness caused by injection which you say was 2 cm. Actually we look for induration , means raised and hard area caused as a reaction to this . If it was really 2cm or 20mm after 48 hrs. , it could not be negative after 70 hrs. then.

Regarding urine report insignificant growth means no organism is showing growth upto the level causing disease and showing any sensitivity to the drug. You may repeat urine routine examination and if showing more than 5 pus cells , you can take a course of antibiotic and take plenty of fluids.
