Brief Answer:
It can be varicose veins

Detailed Answer:
Hello,
I have studied your case. Pain and cramps and dilated veins all are sign and symptoms of varicose veins and it is due to valve problems in vein. I have studied your images but thy were not very clear. I have following suggestions for you .
1. Get doppler examination of the leg to confirm varicose veins or it is a clot ?
2. wear tight stockings can help in dilated veins.

Brief Answer:
Varicose veins can cause pain

Detailed Answer:
Hello,
Varicose veins have bad blood in the leg and does not allow fresh blood to come, so that can cause cramps and heaviness. Also it is possible to have varicose veins in one leg. As you said that you also have Diabetes, your veins may some time become infected due to that.So that can be a complication of Diabetes.
Also there is possibility that metformin is causing this pain. So I have following suggestions for you.
1.you are taking too many medications. Avoid drugs celebrax and Carbazipime. Discuss with your doctor. Some time drugs interaction can cause cramps.
2.hot water fomentation is useful in cramps.
3.take Salt in fluid whenever there is cramp. It will act immediately,
Let me know if their is any other followup questions.
thanks
