Brief Answer:
a lot of things...

Detailed Answer:
Hello,

the information you've provide is not enough to get a very good idea of your grandfather's condition. Some fairly common causes include the following:
- a heart attack which may present with chest pain, low blood pressure, high blood sugar (in patients with diabetes), etc
- dehydration caused by inadequate blood sugar control: high blood sugar causes loss of water, which may cause dehydration and even hyperosmolar hyperglycemic non-ketotic coma.
- an arrhythmia like atrial fibrillation (or a worsening of rhythm control in a patient with pre-existent fibrillation.
- drug side effects
- gastrointestinal bleeding

The list can go on and on. I can't say what's wrong with him because clinical examination, history taking and lab tests are required to reach a diagnosis.

If you can provide more details perhaps it would help me to get a better idea of his condition.

I hope you find my comments helpful!
Kind Regards!
