Brief Answer:
4th degree tear can lead to incontinence.

Detailed Answer:
Welcome to HCM.

Congratulations on the birth of your baby :)

I understand your concerns for your wife.

4th degree tear means there is injury till anal mucosa. This can lead to incontinence.

This is not normal.

It has to be surgically repaired and takes long time to heal.

Along with surgical repair, she may be kept under antibiotics cover to avoid infection.

A semi-solid diet has to be given and take care that she does not develop constipation.

Also, long-term sitting is to be avoided.

Hope this helps.

Feel free to ask further.

Take care.
