Question: Hello Doctor---I am concerned about a recent rx change made for my mother--She is 98 and has been on Plavix and aspirin for about 8 years--She has had 2 strokes-1 in 1994 with carotid artery surgery repair and 1 in 2005--Called a mild stroke--She had heart attack in 2006 with a stent placed in a coronary artery--We got a new doctor that comes to the house(she has 24 hr a day caretakers)--This doctor recommended stopping Plavix and changing aspirin from 325mg to 81mg---All she will take is 1-81mg a day---With all I read about stopping Plavix, I am worried---Can you help me understand this situation---Thanks---XXXX

I have very carefully gone through the details and I feel that indeed that the intervention by your doctor in changing medication makes sense. Doses of 30 mg to 325 mg of aspirin are considered low dose. So, 81 mg would be logical to be given. If aspirin is being given, it is no use giving Plavix as well. Either of the medicines prevent unnecessary clotting from occurring and thereby can prevent strokes from occurring. 81 mg of aspirin should be enough to do the same.

Thank You Doctor for your reply--I was reading about when stopping Plavix, you can have another clotting situation within the first 3 months of stopping it---It said there were mixed feelings about stopping Plavix between professionals---The situations explained were different in that the patient was taken off 1 year after a procedure--She has been on the 2 meds for at least 8 years--In your opinion, the 1-81mg aspirin a day will be good to prevent platlets from clotting

I understand your concerns but even though you are stopping Plavix, she is still under the coverage of aspirin which should be enough to take care of any clotting situation. One 81 mg aspirin a day should be enough to prevent the platelets from clotting.
