Brief Answer:
IOL exchange surgery is last option.

Detailed Answer:
Hello Madam,
Welcome to Health care magic.

Iam Dr. DADAPEER K, an ophthalmologist and I answer health problems related to eye.

It seems from the history that you have undergone cataract surgery in the year 2013 by implantation of Restor Intraocular lens.
Restor intraocular lens is a multifocal lens and it is used to provide good near vision and distant vision after cataract surgery, whereas monofocal lens provides good distant vision only after surgery.

However these multifocal lenses have got few drawbacks in the form of glare and diffraction of light, resulting in difficulty in reading, driving...
Since are also having dry eye following dry eye and you are on treatment with Doxycycline and other eye drops.Dry eyes can also lead to symptoms like blurring of vision.

Hence I advice you to first get treatment for dry eye before planning for one more surgical procedure like removal of intraocular lens.
The treatment of dry eye is by use of use of artificial tears eye drops. The severe cases may require use of punctal plugs or drugs like cyclosporine.

The removal of intraocular lens is not commonly done until unless absolutely required because of risks involved in the second surgery. Though the problems you have mentioned like difficulty in reading are seen sometimes following surgery we try to correct them by medical modes of treatment.
The common cause for these problems is because of pupillary size causing diffraction of light. Hence by reducing the size of pupil by using eye drops or by use of dark glasses will provide relief in some cases.
Hence I advice you to consult your ophthalmologist for evaluation to find out the cause for your symptoms, to rule out dry eye and treatment for it, to find out the other methods of treatment of your symptoms.

If all these treatment options fail to provide relief then you can consider second surgery to replace the intraocular lens.
Any cataract surgeon will be able to do this procedure and hence you can consult your surgeon for further advice.

Hope the information is helpful to you.

Thank you
With regards

Because I have already developed cataract on my Posterior Capsules, I have to make a decision to either keep the Restore Lens or to switch to a monofocal lens because the posterior capsules must still be in place to switch the lens.
Part of the blurring that I am experiencing is caused by the cataracts on the posterior capsules. So how can I know whether I will have better vision and less pain if I change lens? Do you know of patients who have switched to monofocal lens and received relief from the blurring and pain? Have they gotten rid of the circles around lights and the inability to read as well without excessive light?
I do not like the halos around lights, halos which occur night and day around all lights, including reflection from the sun on car windows and on street signs.
Also, do you know how I can find statistics on people who have had Restore lens
replaced with monofocal IOLs? Have people ever lost their vision because of the second surgeries? I need to know what the absolute risks are.
I would appreciate any references you can send me to. thank you, XXXXXXX

Brief Answer:
Pain and burning are not caused by after cataract.

Detailed Answer:
Hello
Welcome to Health care magic

Thank you for the follow up question.

The treatment for after cataract which develops on the posterior capsule after cataract surgery is by yag laser capsulotomy. This cannot be treated by replacement of intraocular lens.

Burning and pain are not caused by after cataract, these symptoms can be caused by dry eye and other causes like chronic inflammations of the eye.

Intraocular lens replacement also called as intraocular lens exchange is a relatively rare surgery and one of the common indication for this surgery is implantation of wrong intraocular lens power. The intraocular lens replacement for problems caused by multifocal lens is relatively rare. The literature shows very few cases on this issue.
The intraocular lens removal does have risks of second surgery like injury to the intraocular structures. These complications are more if the time interval between the primary intraocular lens implantation and the second second surgery is more than 3-4 months as adhesions acquire between the intraocualr lens and the capsule. The problems are damage to the posterior capsule, damage to corneal endothelium resulting in corneal edema...
Hence someone should not hurry into the decision of intraocular lens replacement as the symptoms of seeing halos around the light tend to improve over time. The symptoms of burning and pain in the eyes are not related to intraocular lens or after cataract and these will not improve with intraocular lens replacement.

Hence I advice you not to hurry on this issue but to discuss with your doctor regarding the symptoms and cause for them and to try different treatment options before opting for intraocular replacement.

Hope the information is helpful to you.

Thank you
