Hello Welcome to Health Care Magic

You are a 62 years old male with chief complaints of fear of breathlessness. You have been taking Escitalopram (Nexito) and Clonazepam (Zapiz) since September 2013. Zapiz was discontinued by doctor and you were advised to take Nexito only. Later you assumed that Nexito is addicting and stopped to take the medicine and problem reoccured.

Now coming to discussion-

Nexito is a SSRI group of anti-depressant and anti-anxiety drug. The drug is a safe drug and is free of serious side effects. It is free of abuse potential and it can be prescribed safely even for long time without fear of addiction.

Zapiz is a benzodiazepine drug. It is used in a number of conditions and in your case it was prescribed for symptomatic control of symptoms of anxiety. You have been advised to take drug on SOS basis. The drug carries abuse potential and it is not safe to take the drug for long periods of time. Secondly in elderly patients it can cause impairments in judgement and can aggravate memory impairment if any. So your doctor advised you to stop that drug and continued you on Nexito only.

Now coming to your questions-

- As per my opinion it is better to take Nexito in morning after breakfast. The drug can cause a little activation so it is usually given in morning hours.

- Zapiz causes sedation and calms individual. It is better to take the medicine in evening hours.

- For some time you can take both drugs to improve your symptoms of anxiety. After about 2-4 weeks you can consult your doctor again and can discontinue Zapiz. As per my opinion Nexito alone will control anxiety there after. Keep Zapiz as reserve drug for anxiety symptoms on SOS basis.

I had problem of nose choking at around 2 AM 5 years back and I used to get up from bed and 8walked in my house for 1 to 3 hours. This then slowly aggrevated and was happening frequently. On 12th XXXXXXX 2012 during day time while travelling by car from XXXXXXX to XXXXXXX suddenly I had my first stroke of breathlessness. My brother in law, who was with me gave me 40 ml of antacid and I took 2 shots of Cipla's Foracot.
Then onwards this happened many times. Doctors checked my lung function, did my angiography but everything came normal.
ENT helped me for few months but again same situation and became sucidal.The story further was mentioned in my query.

Usually some times due to broncho-constriction nose chocking can also occur and this results in breathlessness. It is possible that initially it was due to some other cause. But due to severe breathlessness and difficulty in respiration individuals develop the fear of similar complaint occurring again and which ultimately progress to anxiety.

As you have already mentioned that all lung functions came normal, and other investigations came normal so the symptoms are occurring due to anxiety only.

Continue the medicines as I have mentioned earlier, you will see improvement with time. Hope this helps you.
