Question: Hello Dr.
I am at the occasional other case I do not know what it is a state of confusion and forgetting names and often remember after minutes and then I feel tightness in my chest and I became melancholy because I cry.
What is this case and what is the treatment

I have read your query and understood your concerns. You occasionally feel low, tightness in chest, confused and you cry also because of the sad feelings. During that period you also forget names.

There are few possibilities:
1. Anxiety [cause may be stress or unknown]
2. Depression

I would like to ask few questions:
1. Does it occur after any stress? These symptoms are episodic or continuous? Do they last for days before resolution when come?
2. Do you feel panic when they starts appearing?
3. For how long have you been experiencing these symptoms?
4. Do you feel sad most of the time? Do you have lost interest in your work and other activities?
5. Do you wake up early in the morning before the usual time?
6. Does your mood is worse in the morning and get improved towards evening?
7. Do you have guilty feeling inside for something?

So, answer the questions. I shall give further opinion based on your answers.

1-some times and it's not stay for more than minutes but bad feeling yes it's stays for a day.
2-yes.
3.for one and half year.

5.No, but I wake several times after sleeping and I wasn't.
6.The opposite correct when I wake up my mid is better and then on the night it became worst.

Seems to be depression

Hi, thanks for writing again.

According to your answers, in my opinion, this seems to be depression and can be managed easily with drugs.

Drugs like SSRI antidepressants will help you. In my patients, I would have given lexapro 10 mg at night. The effects of the drugs will appear only after 15 to 20 days. You shall find maximum benefit by the end of two months of treatment.

Since, the site does not provide prescription, you have to visit a psychiatrist and ask for treatment. His opinion may differ from mine.

Apart from this, you can also go for:
1. Mild exercise in the morning like walking, jogging etc.
2. Yoga and meditation will help
3. Resolution of the stressful event is also necessary.
4. Engaging in social activities and meeting friends are also good options.
5. Playing indoor game can also help.
6. Take interest in your hobbies and spend time for them.

These all are non-pharmacological methods for management of depression but you will benefit the most by combining them with drugs.
