Question: Hi Dr. XXXXXXX
I am the one who spoke with you on Saturday regarding all the BP medications I am allergic too. You suggested I try PHYSIOTENS. The medications I have tried previously seems to hit my back, butte and down my left leg. When reading about PHYSIOTENS I did notice it said in regards to side effects it can cause back pain. Have you had any of your patients complain about this, and have you tried it on several of your patients? The info says to begin taking 0.2mg/day and then to increase the daily dose to 0.4mg/day if tolerated. My doctor is very interested, and we were wondering what dose to order, and can the pills be cut in half? Thanks so much for your help! It is so much appreciated.

Welcome back.
Sorry for the delay in replying due to the time zone difference.
Physiotens isn't my first line of treatment and I use it mainly for people having side effects with other antihypertensives. The commonest side effect reported by patients is feeling sleepy or drowsy. I don't remember anyone complaining about back pain.
Back pain can be due a number of causes. Your description of back pain is in the same distribution of a disc pinching on the sciatic nerve. You might have noticed the pain worst while using Diavon which is an ARB. My patients on ARBs sometimes complain the same.
Since you have started exercise and lifestyle changes, I hope the pain would be less. Also sleep on firm to hard ( orthopaedic ) bed, avoid bending forward, lifting weight, constipation, cough etc.
Since you may not be getting much exposure to sunlight, it may be a good idea to check your vitamin D level if you have not checked.
Physiotens tablets are very small and not scored. You should not cut them. So your first order can be for 0.2 mg dose which you could start as once a day. If your BP is not sufficiently controlled in 2 weeks, you can increase the dose to 0.4 mg.
