Brief Answer:
NSAIDS group of medicines may harm his kidneys!

Detailed Answer:
Hi! thanks for your query on your father's health issues!

In facts, ibuprofen and aspirins are NSAIDS group of medicines and should be judicially used in this special case of multiple myeloma ...

The dosage should be adjusted according to his blood urea/creatinine levels to avoid kidney injuries or harmful effects on the kidneys.

Therefore, please check his urea/creatinine levels and discuss it with his regular physician regarding proper dosage adjustment and if any alternatives like colchicine/paracetamol etc.

Wishing him good health,

Hello Dr. Samanta,

Last week my Dad's creatinine level was 0.93 and hemoglobin 11.4, but two days ago his left foot/ankle and right hand/wrist/fingers is swollen. We live far from the city, it takes 2hrs before we get to the hospital. I gave him OTC ibuprofen 200mg with temporary relief. Swelling still persist.

Brief Answer:
You may increase the dosage for better response!

Detailed Answer:
Hi! sorry for the delay; however, I must say that his creatinine level was normal and currently though the level is not known, you need to add more ibuprofen up to 400 mg thrice daily after food with ranitidine 150 mg before food but there are better alternatives like etoricoxib (selective COX-2 inhibitors) / steroids and colchicine; Also check his blood pressure and get a routine urine test when possible. To be on safer side, you motivate him for active movements of the joints involved!

Consult his regular physician when you meet in the future to prevent such acute episodes..

Regards,

Thank you for your advice Dr. Samanta,

The swelling mildly subsided but his whole body/bones are aching.
I was thinking about putting Dad on Dexamethasone, was recommended by the Hematologist previously since he declined chemo therapy because his body couldn't handle the treatment. Please advice if it's a good idea knowing his 82 years old on Dexamethasone 40mg weekly. I'm worried about the side effects.

Thank you very much,
Vie

Brief Answer:
I think it is a good idea!

Detailed Answer:
Hi! Dexamethasone is a life-saving drug and always benefits are more if taken judiciously and under close monitoring by his doctor...it has many side effects but the dosage you mentioned, is quite safe for him...!

Only long-term use invites side effects; please try it for him and get in contact with the primary doctor.

Thank you Dr. Samanta, I'll talk to the doctor during his f/u. His primary care provider (Internist) will be monitoring him since they don't have a hematologist in this hospital.

Thank you,
