What Are The Repercussions Of Accidentally Ingestion Of Good Knight Mosquito Repellent Liquid?

Question: Hi doctor, my daughter accidentally drank 5ml of good Knight liquid.. I taken her to near by hospital.. they said she need to be in absorvation for 12 hrs and then only they can give treatment and they took one x ray... Seeing that x ray they told that she is having infection in her lungs.. but no treatment was provided only rantac IV fluid was going.. so I taken her to my regular pediatrician for consultation.. he said only an antibiotic was needed.. I given her that for 5 days.. once the course was stopped,she is having high fever and cold also... She is crying the whole day...but her pediatrician was saying it's only Viral fever and there is nothing to worry.. but am very much disturbed... I read in an article that there will be highly chances of pneumonia for her.. is that so... Plz... Tell me..

I've gone through your query and understand your concern for your daughter..

I'd like to appreciate your reading, as it's quite true, after this type of poison(insect repellent) ingestion, due to their volatile nature, there's pneumonitis but that's mostly chemical pneumonitis dear, so ideally antibiotic therapy is not required in this scenario..

The hospital authority was apparently right in their approach as in initial stage only supportive treatment is required in such insecticide poisoning cases, until any of the investigation shows any thing significant or there's any significant symptom exhibited by the patient..

Can you please clarify the following so that I can guide you more specifically :

Your daughter's age and approx. body weight ?

Was there any vomiting after the good knight ingestion ?

Please specify which antibiotic therapy was given along with dosage ?

Have you recorded the fever, if yes can you please let me know the exact temp. ? how is the trend of the fever ? is it continuous/ on-off ?

Is she coughing ? any mucus/phlegma with cough ? if yes what is the color ?

Any other problems along with ? is there any breathing difficulty also ?

It can be viral fever later on, but after an exposure of insecticide ingestion like this, chances of chemical pneumonitis is always more.. Chest X-ray(repeat) PA view is required and if in doubt, HRCT chest may need to be done for further evaluation..

Fever is mostly seen due to the inflammation in the respiratory tract, it needs to be controlled, so please give her Syp Paracetamol as per her body weight three times a day(10 mg/kg/per dose) after food for next 3-4 days..

However, please don't panic, it mostly gets resolved within 2-3 weeks along with other supportive measures (keeping her away from cold air, giving steam inhalation three to four times daily, maintaining good hydration, let her have plenty of warm liquids like soups, porridge, broth, bournvita etc.).. however if there's a lot of secretions during cough then she'll be needing saline nebulization along with decongestant therapy and/ bronchodilator/steroids after the imaging examination..

I'd suggest you to get her checked by a Pulmonologist (preferably pediatrics but if unavailable then adult only), for a complete respiratory system check-up and analysis..

I'll be glad to clarify further, let me know all those answers which i've asked too.. also please let me know if any further investigations after the hospital one (eg. any repeat Chest X-ray) is done already, if yes then how long back..

Thank you Dr for your quick response.
My daughter is 19 months old..her weight is 9.9kg..
She dint vomitt.. she was very badly coughing...in the hospital we admitted her,she was in ICU , they dint give any antibiotic... They have given only IV fluid...
And taken a chest x ray...
Since the hospital atmosphere was not neat and good..no response from them after 12hrs also.. we taken her to my regular pediatric Dr.. he only given antibiotic..moxclav and common cold syrup...
After 5 days of antibiotic.. she got high fever like 102 degree and stop after given calpol but back again in 5 to 6 hrs..
She is coughing badly... No mucus in cough.. running nose and no color in it...
She had breathing issues when we admitted her there.. now she is breathing through mouth.. her pediatric Dr told it's bcoz of cold and viral fever and it's no were related to effect of mosquito liquid...

No post x ray was taken...
My daughter is suffering badly... She is always crying also....
Dr, is her situation very bad?? Am really scared...

Thanks for following up with the details,

The breathing issue mostly is a combination of inflammation of the respiratory tract due to the toxins along with probable secondary viral infection..

Please have faith in your treating doctor, but if there's doubt then you can always opt for a second opinion(which I've already mentioned)..

Yes, you can give her steam inhalation which will relief some of the symptoms..

You can observe the symptoms and if you feel that they're increasing then you can discuss with the doctor regarding further increment in the line of management (see my previous reply where it's mentioned regarding nebulizations and other measures).. Co-Amoxyclav(MoxClav) is one of the best antibiotics given to those situations, so don't worry..

A follow-up X-ray will be very useful as it'll give further picture of the lungs and disease progression.. and the mouth breathing may or may not be related to mosquito repellent(in fact it can be a combination of both the toxin's effect as well as secondary infection),please check for any nasal blockage also.. confirm opinion is possible on that only after examining her clinically along with investigations..

Don't be scared dear, believe in your treating doctor, and like I've already mentioned , if still in doubt, just go ahead and have the second opinion, as it'll give you further clarity of the situation..

Thank you Dr... My daughter is very active girl.. she won't sit once place.. everyone use to say tht she is hyper active... Her pediatrician is a good Dr here.. thats y we taken her there... I have very bad opinion about the hospital where I admitted.. not clean,no response,treating rude,even a kid in ICU..
Other than tht I trust my pediatric Dr...

Seeing my child, as a mother I couldn't be brave..she is very dull and crying always... I never seen here like tht.. my home atmosphere itself is changed. missing her charm,naughtiness and smile,Which bothered me a lot.... thanks a lot for it kind reply...
