Question: Hi Doctor,

I am 26, me and my gf were doing pleasure to each other by touching each others private parts, during this process,her hands got sticky with my sperms,even i played with my self and got sperms on my hands aswell,then i fingered her vagina with the infected(sperm) hand for a while. we did not have any sex at all.Just after about 40 min she went to the bathroom,she noticed her vagina was wet and she touched her vagina(fingering her self) with the infected hand that had my sperm on it and then she took bath. This event(pleasuring each other) happened on saturday,27 sep at 3 pm and she took the ipill on monday,29 sep, she had the ipill around 11:30 am  on monday which is with in 48 hours. Sir i am concerned,will she get pragnent.

General information:
Her periods are of 5 days every month, she will have heavy bleeding on 2 and 3 day of each period every month. She will have light bleeding on 1,4,5  day  of each period every month.

Her period dates for July, Aug, sep are

19July-23july
28 aug- 1 sep
22 sep-26 sep

Doctor I would like to tell u that the event of pleasuring each other happened on 27 sep which is just the next day when her periods got over(22 sep- 26 sep).  Doctor I am concerned, will she get pregnant

Thanks for contacting health care magic.

Firstly, Ipill has to be taken within 72hrs of intercourse as an emergency contraception pill, which you have duly taken.

Secondly, your gf was in her safe period on 27th sept. (proliferative phase of menstrual cycle), when ovulation had not occurred. So the chances of getting pregnant is negative.

Her cycles have been regular. Sperm life is also 72 hrs so if she has fingered herself with smudged hand then even if sperms have gone into vagina, she remains safe of pregnancy as she was in her safe period and as a precaution taken Ipill well on time. So rest assured, she should not get pregnant.

Just that she may experience some spotting after 5-7 days as withdrawal from Ipill depending on the endometrium thickness on the early proliferative phase of the cycle. She may not have a normal menses this time but they might be scanty or just spotting.

If at all she doesn't get her periods, which is also likely then she can wait for her next dates and need not take another Ipill or any other drug.

Also would like to advise to follow barrier contraception in form of condoms during her this cycle.

Hello.

Thanks for the follow up.

Her periods have been regular and as I said earlier as well that the act was performed during the safe period and as you said on the very next day of stoppage of menses when the endometrial lining is very thin and unfavourable for implantation. Also as a precaution she has taken Ipill to prevent fertilisation and implantation.

In a way its double safety. So, I don't think that she should be getting pregnant in this cycle.

Thanks for the follow up.

So as predicted this bleeding is the withdrawal bleed of i pill. this may range from spotting to minimal bleed ( 2-3 sanitary pads per day) , lasting for 2-3 days.

This rules out pregnancy as well. now only thing is she should count 7th October as her fresh menstrual period date.
She may pass some clots but that's normal. Get back to me if in case the bleeding is heavy and not getting stopped in 3 days.

thanks for follow up.

please dont worry, the bleeding is as expected . its the withdrawal bleeding following ipill intake. 2nd and 3 rd day bleeding is likely to be heavy as in 4-5 sanitary pads per day and then should reduce.

pregnancy is ruled out as menses have started. so rest assured. just watch the bleeding . if in excess , beyond 7-8 pads , then we can consider antifibrinolytics to reduce the bleeding . but i think it wont be required. offer her rest and hydration and also hot fomentation for lower abdomen, that should help her soothen the pain.
