Question: i was w/ a transexual sex worker. we had anal sex (i was receiving) she had small penis and condom may have slipped up her shaft a bit during intercourse, but never off. she came inside me but in the condom and properly pulled out keeping the condom on until she was complete away from me. if there was some leakage at the base of the condom where it was slipping up, What are my risks of hiv?

Thank you for query. I am Dr Bharatesh D Basti answering your query.
As the condom is intact and there was no contact of the secretions of either of the partner, there is least chance of transmission of HIV from this act.
If the partner status is negative for sure you need not bother.
The HIV status of partner is unknown i suppose.
I feel there is unlikely of secretions to come in contact as small penis.
If still there is chances of contact of secretions of partner ,then there is a risk as anal sex has slightly more risk than the vaginal route (The risk from vaginal route is 1 in 200 to 1 in 1000. ).Hope there are no abrasions and ulcers in the anal area.
If so you can consult your doctor and you can get post exposure prophylaxis if within 3 days of intercourse.
later you can get tested for HIV.

she said she has no diseases but did not divulge the last time she was tested. it has been more than 3 days. i didn't notice any ulcers or abrasions on my partner and i have none. she came in the condom and due to her small penis i noticed the condom slid up a bit. could it have leaked out the bottom? the actual orgasm was in the condom but i was worried that because the condom was sliding it may have been loose and pre cum or some cum may have been able to come out of the bottom of the condom while still on. I didn't feel like there was anything inside me after she finished. lube was used. and i saw her her hold it as she pulled out.

If the condom is intact and there was no contact of the secretions of either of the partner, there is least chance of transmission of HIV from this act.And you can allay the fears of HIV totally.
If the partner status is negative for sure you need not bother.
There is less chance for fluid to escape from condom and enter anus if intact.
The risk from unprotected vaginal route is 1 in 200 to 1 in 1000 if the person is positive.
it means that chance of getting infected is 1 in 200 to 1000 intercourse if one of them is positive.It is slightly more in anal intercourse as there are more chances of abrasions ulcers or wounds or rough intercourse.
If protected chances go towards nil almost.
You can allay fears of HIV if no contact with fluid whatsoever.
