Brief Answer:
Its possible to detox without rehab

Detailed Answer:
Hello,
Thanks for choosing health care magic for posting your query.
I have gone through your question in detail and I can understand what you are going through.
I understand that you have multiple issues:
1) H/o incest
2) Borderline personality issues
3) Opioid dependence
4) Joint pains

The codein per se cannot cause joint pains but its withdrawal can definatley cause joint pains. Further the common causes of such joint pains at the age of 61 years is arthritis. You can get the arthritis evaluated bygetting an x-ray done along with some blood tests like RA factor, SLE test.
Regarding coming of the substance abuse, what I suggest is you can have a gradual decrease in the opioid dose that you are taking. This way you will have a least amount of withdrawal. Further there after you can try drugs like baclofen and naltrexone which can help you in rehabilitation phase. Discuss all about this your doctor.

Hope I am able to answer your concerns.
If you have any further query, I would be glad to help you.
