Question: I have orthostatic blood pressure, meaning it is 180/110 prone and 90/50 standing. Because of this my body tries to eliminate fluid an therefore dehydrated causing a syncope effect. I need to figure out how to lower my resting blood pressure?

Detailed Answer:
Hello!

Welcome and thank you for asking on HCM!

I understand your concern and would explain that your symptoms seem to be related to an autonomic dysfunction.

It may be related to the brain radiation, causing a central autonomic dysfunction or to some hypertensive drug intake.

Unfortunately there is a big dilemma in treating this disorder, as the drugs which help reduce high blood pressure cause aggravation of orthostatic hypotension.

The most important thing to treat in this case is orthostatic hypotension , as it may be dangerous causing repeated trauma from falling.

Some tips would be helpful:
-avoid sudden head up postural change
-avoidi standing still for a prolonged period of time, prolonged recumbence during daytime
-avoidi straining during micturition and defecation, hyperventilation
-avoid high environmental temperatures, severe physical exertion
-avoid large meals especially with refined carbohydrates, and alcohol
-avoiddrugs with vasodepressor properties.
-Even simple water intake increases have been shown to be beneficial.

In addition, exercise of leg and abdominal muscles, especially swimming, may be useful.

There are no evidence based guidelines to treat this situation.

But I would recommend considering the use of clonidine in low doses may be helpful in treating this situation coupled with nitroglycerin tablets only during the night (as you are going to stay in laying down and the possibility of high blood pressure during sleeping).

You should discuss with your doctor on the above mentioned issues.

Hope to have been helpful!

Best wishes!
