Suggest Ways To Control Diabetes

Question: hi doctor,

my self XXXXXXX
suffering from diabetes since more then 6 years.
but still fasting sugar is 180-220.
and taking triglycerides 2 twice in a day before breakfast and dinner and one tablet of zomelis met 50/500 after lunch.
kindly suggest

Noted your concern.

Please confirm the name of Tab Triglyceride 2, I am not aware of any such medicine.
But my interpretation regarding it is a combination of Glimepiride 2mg, Pioglitazone 15mg & Metformin 500mg.

Tab Zomelis - Met contains Metformin 500mg and Sits glinting 50mg.

In total you are on 4 different medicines for diabetes almost in a maximum effective dose of each.

In such a case I may suggest
- Continue taking Tab Trigl.... 2 twice a day.
- Increase Tab ZomelisMet 50/500 twice a day.
- Recheck fasting & pist-prandial blood sugar levels after 2 weeks.

If it is still not in control then addition of following medicine to above mentioned combination may help
- I may prefer use of long acting insulin like Insulin Glargine single dose at night time.
- In patients not willing for insulin, I may suggest use of tablet containing Canagliflozin or Dapagliflozin.

Along with this you should follow diabetic diet & regular exercise.

Whatever above treatment options mentioned are considering normal kidney & heart function.
All these options need to be discussed with your treating diabetologist.
