Question:
Hey Doctor,

I had sex with a girl on April 5 2016 which was the day before the start of her LMP. She actually started bleeding (SPOTTING) on April 6,2016 . She had an ultrasound done on XXXXXXX 2 2016 which stated she was 8 weeks and 1 day pregnant. She also mentioned that she had been having sex with another guy around April 18-27 of 2016,but said she had protected sex with him however I don't know If that's true. Can you explain to me what are the chances she was conceived . I am unsure if she got pregnant by me or the other guy. I use the pull out method but I don't know if that was enough . Today I believe she is 15 weeks and 6 days pregnant from her last LMP. Also she is saying her doctor said she got pregnant the week of of her LMP is that true? From my understanding , the fetal age is usually 2 weeks less than her gestational age. If this is true how can this possibly be my child if we only had sex the day before her LMP which was April 5,2016. She also said the doctor gave her the expected due date of January 11,2017(40 weeks) . Overall based on all the information given what could have possibly happen wrong in the ultrasound scan and or the information given for her to believe the child is mines and not someone else. I am basically asking what are the chances this could be my child based on the facts of the information given in the scan and whats is given in the discussion. Can the fetal age and gestational age be the same or that close in the week? Have you heard of any special cases where this could be possible? I need to ensure that I am not the father of the child? Please see the attached ultrasound scan in the update report section of this discussion.

Thanks

Brief Answer:
Conception occurred around 4/20

Detailed Answer:
Thank you for the question. I will do my best to clarify what is going on from the information that you have provided. Counting the first day of a woman's period as day #1, most women ovulate around day #14 and get their period on about day #28. Having intercourse on day #27 is unlikely to produce a pregnancy. Also, we include the first two weeks (before conception) in the 'dating' of a pregnancy - yes it is confusing. That being said, the ultrasound (I did look at the attachment) actually provides accurate information regarding conception. Based on the following:

1. On 6/2 ultrasound = 8 2/7 wks
2. This corresponds to a last menstrual period starting on 4/6
3. Due date based on the scan is 1/11/2017
4. CONCEPTION OCCURRED RIGHT AROUND 4/20

The ultrasound has a maximum 'window of error' of +/- 3-5 days. Taking this into account, conception occurred sometime in the window of 4/15 - 4/25. Based on this information, it is impossible that you are the father. Obviously Guy#2 did not always use a condom. If she is claiming that you are the father then it is as simple as asking for paternity testing after the birth to prove that I am right.

Good luck!
Dr. Tim
