Posted on Tue, 29 Apr 2014
Question: I am facing work-related anxiety- always thinking that I am not doing well at work. This is adversely affecting my concentration level. Also, I have fear of public speaking and find it difficult to explain to co-workers on phone. I struggle with these problems on a day-to-day basis. Please let me know what therapy is best suited for my case.

Brief Answer: Hi Detailed Answer: Hello, Thanks for your query. I appreciate your efforts for medical consultation in so much distress. From your query it appears that you have anxiety disorder. Anxiety disorder may occur in form of generalized anxiety disorder (some anxiety all the time without any reason), social anxiety (exacerbation of anxiety in social situation and avoidance of social situation), panic disorder (sudden attack of severe anxiety) or combined form of these disorder. Symptoms of anxiety which may or may not be present in your case: nervousness, restlessness, mild irritability, excessive worry (regarding own health/finance/family), tremor, muscle spasm, heaviness in body, easy fatigability, headache, sleep disturbance, palpitation and dry mouth. Anxiety occurs due to neurotransmitters imbalance in brain most commonly involving serotonin neurotransmitter. Due to that medicines which stabilize serotonin transmission have good effect on anxiety problems. There is need for treatment, if symptoms cause significant personal distress or affect personal/professional life like in your case. Both drug and non drug treatment like cognitive behaviour therapy (form of psychotherapy) are used for treatment. Choice of treatment depends on severity of symptoms, personal preference and availability of treatment. In general combined treatment (drug treatment with psychotherapy) is best form of treatment. In drug treatment two types of medicine used. For immediate relief of anxiety benzodiazepines group of medicines are used and for long term relief most commonly SSRI (selective serotonin reuptake inhibitors) are used. SSRI takes around 3-4 weeks to show its effect. Non drug treatment like cognitive behaviour therapy (form of psychotherapy) also takes weeks to show its effect. So take first step and meet nearby psychiatrist for treatment. Apart from above advice following simple measures will help you to some extent: -Healthy diet: Milk and fruits in diet. -Daily morning walk -Follow structured daily routine. Fix sleep and wake up time. -Do not use excessive caffeine (tea, coffee and cold drink) -Stop substance use like alcohol (If present in your case) -Breathing exercise, yoga and meditation (which you can learn from some nearby expert) I hope this information has been both informative and helpful for you. In case of any doubt, I will be available for follow ups. Wish you good health. Regards,
