Question: My 18 YEAR OLD DAUGHTER HAS BEEN VOMITING AND HAVING DIARRHEA AT NIGHT. SHE SEEMS TO HAVE GERD SYMPTOMS FREQUENTLY BUT HAS HAD ENDOSCOPY AND TOLD THERE IS NOTHING THERE. HER DOCTOR FEELS IT IS SOMEWHAT EMOTIONAL. SHE HAS HAD A COUPLE OF UPSETS OVER THE HOLIDAYS. I AM A NURSE, BUT DONT WANT TO IGNORE ANYTHING. THIS HAS BEEN GOING ON FOR 2 DAYS. SHE DOES NOT SEEM WEAK, IS HAVING BOWEL MOVEMENTS, AND IS NOT RUNNING FEVER. SHE IS HAVING LAPAROSCOPY ON THE 12TH TO CHECK FOR VARIOUS ABDOMINAL SYMPTOMS AND POSSIBLY ENDOMETRIOSIS. SHE CANNOT SEEM TO KEEP ANYTHING DOWN. SHOULD I BE DOING ANYTHING OR JUST WATCHING HER CLOSELY?

Hello,

Thank you for using HCM to post your daughter's health issue.

I read your query carefully and I understand your concern.

Your daughter is of an age that not many dangerous problems are to be expected.
From the information you have given me, I would say that the most common problem your daughter might have is a viral gastrointestinal infection. GERD usually doesn't come with vomiting.
Anyway, if her story is longer then 7 days with vomiting, then it is not a common viral, and of course needs to be investigated.
If endoscopy is negative, then GERD is even less likely to be, because endoscopy is the best test to diagnose it.
On the other side, to think about other diagnoses, your daughter should have other symptoms- for example if irregular periods and strong pain in menstruation we think of endometriosis.

If I was her caring doctor, and with the information I have now, I would advise:
-- Make sure she drinks plenty of fluids and remain well hydrated- small amounts and often. Oral rehydration salts may help.
-- Is she sexually active? If yes, consider a pregnancy test.
-- For the diarrhea, put her on a diet with rice, yogurt and banana. Avid milk, because makes diarrhea worse.
-- If the pregnancy test is negative, she can try Metoclopramide 10mg, 3 times/day against nausea and vomiting, and Loperamid 2mg, if she has lose stools more then 4-5 times/day. Use loperamid as 1 tablet after every lose stool, maximum 8 tablets/day.
