Suggest Treatment For Vitiligo In An Elderly Person

Question: Hi Doctor !
I am 67 years old Male and since 7 years now I have a vitiligo disease (which starts age 60 ) .
Initially, the small white patches appeared on the private areas on my body (wrist area, groin area and my armpits ) and now its slowly spreading .
White patches appear symmetrically in the same areas on my body. (for example: both side of the left and right hands )
I have consulted with skin doctor and try couple different medicines such as ( MELGAIN PLUS 25 ml made in Cuba ) and (MELANOCYL-OINTMENT 25g) seems like a treatments not helpful and I want to change the treatment. Please suggest what I should do ?

Hi
Thanks for being at healthcaremagic
I have gone through your complaints as well as reviewed the attached photographs.
You are having symmetrically progressive vitiligo for which I recommend my patients to take oral medication along with topical medicines.
If I was your treating doctor I would recommend you to take oral steroids like tablet betamethasone 0.5 mg per week body weight per day on 2 consecutive days in a week.
This needs to be continued for a long time but under the supervision of the dermatologist.
Besides this I would recommend you to apply tacrolimus 0.1 ointment on the affected area once daily at night and clobetasol propionate cream on the affected areas in the morning.
These are prescription drugs and you need to see the doctor in person to get them.

Could you please explain me: What was the reason of appearing vitiligo on me. what means if person has vitiligo in simmetrically.
and may you tell me, Does this vitiligo can be cured out, or not.

Vitiligo is an autoimmune disease, can occur at any age in any individual

Detailed Answer:
Vitiligo is basically an autoimmune disease which can occur in any individual at any age. It is basically related to destruction of the pigment producing cells by various mechanisms.
It has been seen that 10 percent of the affected individuals have a positive family history.
Even symmetrical distribution of vitiligo is very common and there are no specific reasons for it.
Vitiligo has an unpredictable course in different individuals.
Looking at your photos their is a possibility of it resolving but with a prolonged supervised treatment.
The main aim of treatment in initial stage is to stop progression followed by repigmentation of the patches.

Do I need to keep diet while treating? can I eat Bitter or sweet foods?

Take a balanced diet. No need to go for dieting.
No need to avoid any foods.
Yes you can take bitter and sweet foods.
