Brief Answer:
intermenstrual bleeding...

Detailed Answer:
Hello,
Thanks for the query to hcm,
The most common cause in your case, with onset of hot flashes at the age of 51 is more indicative of you setting into perimenopausal stage. Menses tend to be irregular and often inter menstrual bleeding is experienced. But a single episode per se is not enough to make a diagnosis but to rule out perimenopausal stage, you need to get a hormonal profile with FSH, LH and AMH levels done. Higher fsh and lh levels shall confirm perimenopause and decreased AMH shall indicate decreased ovarian reserve.
Also to be considered is the role of cymbals which is an antipsychotic and has a role in hormonal imbalance per se causing irregular shedding during the mid cycle. Hypothyroidism if uncontrolled can lead to delayed menses so at present with you being on synthyroid, this seems unlikely.
Mild bleeding can be seen after a pelvic exam if there are cervical erosions present over the cervix causing frictional injury and hence bleeding. So ideally a pap smear exam should have been done before the same or a per speculum exam should be considered for ruling out any cervical ectopy.
you should discuss the options with your physician in detail and take an informed decision accordingly.
