Suggest Treatment For Urinary Incontinence During The Night

Question: I am 39 years old married women. I got married in 2007 and I have two children. First son was born in 2008 and second born in 2012. Problem started after childbirth of second child. I pass on urine in sleep. It doesn't occur frequently. It happens once in a month or sometime once in two months time. Like tonight I awoke when urine started passing but strangly it didn't stop passing even when I awoke. I have good bladder control during daytime.

My mother started facing this problem after menopause. Her problem occurs during daytime too. She is diabetic too. Although I am not diabetic but my suger level touches higher limit if get tested. My height is 5.3 feet and I am 72 kg. Both my child deliveries were by normal/vaginal delivery only. I am working women and have long sitting hours in my job. Sir What do you suggest/diagnose from these symptoms?

I have noted your clinical details. Based on that, your symptoms are most probably related to weak pelvic muscles. It does not sound like any major neurological problem, as in that case, urinary incontinence (urine coming out without control) would be seen in day as well as night. You should consult a gynaecologist, and learn some exercises to strengthen pelvic floor muscles.

Sir I forgot to mention one fact that about 22 years ago I was diagnosed about Spine Bifida. I was told that its mild one. I cant tell you about the Level at this time. At that time I was treated for Bone TB for one and half years. It was cured. I was advised to take precautions like not to bend, not to sit at hard surface. Today in the early morning when I wrote to you I woke up when I passed urine in sleep. Today in the daytime also I felt leaks. Although I have good bladder control but still sometime I face leaking problem but not while sneezing or coughing. As I told you at the time of childbirth I started facing this problem, at that time My gynecologist told me to consult neurologist. Will you still advise me to consult gynecologist or I can consult neurologist too? please advice.

I have noted the additional details. In view of past history of spina bifida, you should also consult a neurologist. He would examine you clinically and if needed, an MRI of lumbar spine may be needed.
