Question: I have upper arm pains in the evening even before I take my medications. Tylenol doesn't seem to help alleviate the pains in both arms.

I am sorry for the painful situation you are going through.

You say to feel upper arm pains in the evening even before you take your medications. Here I need to ask about the medications you are currently taking: is it Tylenol only or other meds you take on daily basis? any chronic illness (diabetes? cardiovascular problems?)

From your description, the pain you are feeling could be related to arthritis or spondyloarthrosis. Xray of vertebral spine neck and chest area and shoulder joints is necessary to rule them out.

Blood tests for arthritis are also of help.

Check them out with your doctor.

Meanwhile, for the moment, I advise to:
- apply analgesic cream on the shoulder joints and neck area.
- apply warm compresses locally; hot tubes can help
- massaging the affected area can also be of help
- if ibuprofen is not helping, should take other more potent anti-inflammatory medications like naproxen or oxycodone, but need prescription from your doctor first.
