Suggest Treatment For Unilateral Hydronephrosis

default
Posted on Fri, 17 Apr 2015
Question: Thankyou!
My brother is an XXXXXXX who is living in Hong Kong and was just diagnosed with severe (acute?) hydronephrosis in one kidney. The doctors have done multiple cats cans and other tests and can't seem to find out what is causing it (no detectable stones or cancer or..). If he were your brother what would you recommend he do (or ask?)

Brief Answer:
need some details to guide you better

Detailed Answer:
Hello

My name is Dr Muhammad Ali Rauf & i will look into your problem.

I have gone through your query and would try to help you in the best possible way. Most common causes of unilateral hydronephrosis include:
- Obstruction due to calculi
- vesicoureteric reflex
- Cystitis
- Neurogenic bladder

> Please share reports of (if done already):
- CT-KUB
- Ultrasound KUB.
- Urine C & E
- IVP(CT or X-ray)

Is there any history of:
- painful urination
- non smooth urination
- burning sensation during urination?

I will be looking forward for the answers to conclude the discussion in the best interest of yours.
Thank You

Thank you but this is none of those. There is no fever or other sign of infection. No kidney stones or tumors visible after several CT scans. The doctor is hypothesizing that it is in the category of VTJ obstruction and wants to wait a week or so to see what happens.
If this were your brother what would you do?

Brief Answer:
Vesicoureteric reflex hold significance in such case

Detailed Answer:
Hello

thank you for getting back. In cases where no obvious cause like obstruction is identified, vesicoureteric reflex holds an important significance causing unilateral hydronephrosis. It can be confirmed by undergoing micturating cystourethrogram. We need to confirm the patency of genitourinary junctions and pelvicalyceal system. I would suggest you to discuss this test (micturating cystourethrogram) by getting an appointment for a "Consultant Urologist" specifically.

Hope I have answered your query. If you have any further questions I will be happy to help if not then don't forget to close the discussion and rate it.
Wishing you good health !!

Thank you for your response Dr. XXXXXXX Rauf.
I am surprised that the query was not given to a specialist in urology, but perhaps your team is not made up of specialists?
At any rate, I appreciate your input and we can consider this matter closed.
sincerely,
XXXXXX

Brief Answer:
You are welcome

Detailed Answer:
Hello

You are welcome.

Such procedures require one to one contact (between doctor & patient) because of detailed discussion regarding procedure. Also before doing a procedure, doctor will thoroughly examine all the tests/investigations done previously. All of them help in reaching definitive diagnosis. Even if a specialist answers would have asked you to consult your local doctor because you would be requiring prescription and written recommendation for procedure. Both these can not be given online due to some rules & regulations. Here we guide patients and redirect to concerned authorities and if possible give OTC medication along with general tips that can help treating the problem. In your case your brother requires procedure for evaluation so i advised you to get an appointment for a urologist. At the moment it would be unnecessary to prescribe any type of medication due to which reason i did not advise any.

Hope i was able to clear the point. I'll be available 24/7 for if you need any help. Don't forget to close the discussion and rate it.
Wishing you both good health !!
