Question: I've had this tiny rough spot near my tooth for years. no pain, or anything. I can only feel it with my tongue. what do you think it may be? the blood is from my scratching my gum right under it.

Read your query and I have gone through report (image) you have attached as far I have observed these tiny rough spot in gums are due to periodontal problem .
You have periodontal pocket in premolar tooth and in first molar you have initial caries .

There is nothing to be worried so much, For this I will suggest to consult oral surgeon for oral prophylaxis (scaling and curretage) and visual examination if needed go for IOPA Xray of molar tooth if it is carious go for restoration and you can also do some home remedies :
-Maintain proper oral hygiene by brushing twice daily .
- Use chlorhexidine mouthwash .
-Do warm saline rinses 2 , 3 times a day .
- You can apply ointment on gum like Stolin gum paint for inflamation.

Read your query as you are asking for patch this can be due to bacterial plaque deposition on sub gingival areas of gums. See what happened is bacterial plaque causes inflammation of gums, gums become red and gum start to pull away from teeth leads to exposure of root and periodontal pocket formation.

There is nothing to be worried so much I will suggest you to go for oral prophylaxis and maintain proper oral hygiene.
