Brief Answer:
need to investigate for phemosis

Detailed Answer:
welcome to HCM.
I can understand the distress you must be through.
Considering your compliant of tight foreskin, the first condition to consider is of phemosis.
It is the condition in which the foreskin especially opening is constricted or tight. The resultant issue is difficulty in retracting the foreskin. The problem is best manifested during masturbation or sexual intercourse when the penis is erect.
It could be primary or secondary to trauma or scaring to foreskin.
The good news it can be managed successfully. However, the management part is best discussed after assessment of condition and its severity.

For establishment of a condition i would need to know from you
1) has it happen that the foreskin got stuck on the shaft and is unable to slide back or forth ever?
2) How much retraction is possible?
3) when and how was the problem first noticed by you?
4) how is your experience with masturbation? if?
5) a photograph can best provide the information, kindly sent if possible.

It is advisable to consult and get it examined by a local surgeon, as there happen a need for physical evaluation.

Kindly reply on above questions.

Dr Pankaj

I have tried to pull the skin during erection and it got stuck once or twice. But dont think there is any scarring. Once erection subsided skincame back
During errction no retraction. Otherwise normally skin goes back properly with one tight attachment at the bottom of the head of penis.
Even duting masturbation skin doesnt retract but otherwise normal..
Attaching a photo of skin attached at the bottom when no erection.
Please advise

Brief Answer:
frenulum breve

Detailed Answer:
thanks for the reply.
the picture you have sent is very helpful. The finding are suggestive of a small chord of skin on the undersurface of shaft. Possibly it is this tied skin that is preventing you from able to retract the foreskin completely.
It is totally treatable, a small surgery of incising the skin chord and suturing is done.
However, a consultation to local surgeon is highly advisable, as it warrants local and physical examination to assess the severity.

There is no need to worry a timely management can help you function normally.

welcome for future suggestions.

Dr Pankaj
