Question: hi
for the past 6 8 mothes i have a hard time breathing only in the evening night.i work out and surf and never have a hard time . 3 monhes ago i was diagnosed with GERD.i had an endoscopy and he said i have acid and an hernia.(that was a year ago)i started emprozole 3 months ago.its better but still have attacks i also take zantac in the past 3 months i had 2 ekg for the heart a chest xray and a breathing test just to make sure and all was good. i feel that when i dont have gluten for a while i feel better.im under alot of stress because i was diagnosed with HIV 2 years ago (my numbers are great im undetectable and good cd4 i take blood test every 6 months so im on top of my health)im just going crazy with this breathing thing. i never have pain and it never wakes me up i just feel like my esophagus is tight. i felt good for the past week and last night again i had a small attack.i dont watch my diet 100% its hard for me to do the small meal thing im not use to it.thank u for ur time

Thanks for posting query at HCM.

Your current presentation ( breathing symptoms at night) especially in the background of Hernia/GERD maybe related to acid reflux.

You maybe familiar with the common mechanism of gastroesophageal reflux disease (GERD) ; however i will briefly explain:

Food enters in to stomach from esophagus via lower esophageal sphincter (LES) . If XXXXXXX fails to close/contract effectively food content from stomach may reflux into the esphagus leading to GERD.

Hernia and Certain food may lead to relaxation of XXXXXXX and/or increase gastric acid production. :
1. chocolate 2. caffeine. 3 Alcohol. 4. fatty meal. 5. citrus fruit and tomato

The process may result in damage to the inside of the esophagus caused by "longstanding" acid reflux disease, called gastroesophageal reflux disease (GERD).

Zantac is a good option but Nexium OR Ganaton is better. You may take of the latter drugs once daily before night meal and observe your symptoms for few weeks.

Further advise is:
1. Avoid chocolates; carbonated drinks,
2. Avoid tomato juice; citrus fruits
3. Minimum alcohol intake and try quit smoking ( if your a smoker)
4. Eat your dinner at least 2 to 3 hours before going to bed ( as you mentioned you cannot have small meals)
5. Raise your head by placing two pillows under your head before sleeping
