Question: My Father is diagnosed recently with CKD, his age is 77, diabetic from 18 years, taking 48 units insulin, his creatnine was 5.6 in XXXXXXX 2014 and was taken to emergency as he was unable to pass urine, then it was concluded he is with CKD, one kidney is shrunk and one was infected and functioning low. During treatment in Jan-Feb he also went thru dialysis during hospitalization and post for 6 times, 8-Feb-14 was his last dialysis, after that he is okay without dialysis. His current creatnine is 5.0 varies in the range of 4.4(lowest) to 5.7(highest), he is asked to take cudo forte, as of today he has completed 1 month of cudo forte. After taking the cudo forte his creatnine went from 4.4 to 5.0 (Mar 2013 - Creatnine was 1.8) what we should do in this case? he is passing urine regularly and okay, sometimes in evening some swelling in his legs but it is mild and dr suggested to take LASIX(1/2 tablet) - Any suggestion or help whether the diagonsis is on right track ... pls review we want to lower his creatnine because last year Mar 2013 it was in range of 1.4 to 1.8

Brief Answer: Continue the prescribed treatment Detailed Answer: Hi, I appreciate your concern for your father.I have reviewed all the records and case summary you have attached and I feel your father is under very good care and treatment. After suffering from Kidney infection (Acute Pyelonephritis) in XXXXXXX 2014 ,he has recovered well and is now without Dialysis since then . CKD is a slowly progressive disease ,in which there is loss of kidney function over time.Gradually over years ,nearly all patients require some form of Renal Replacement Therapy.Your father seem to have recovered well ,since his kidney functions are stable with Serum Creatinine around 5.You can plan for timely placement of a permanent access of dialysis(if required in future) like you arrange for surgical creation of arteriovenous fistula,if possible . Meanwhile what you can do is to take care of his Total Intake and Output. Monitor closely his urine output.Watch for any acute decrease in urine output. Watch for any pedal edema, breathlessness, altered sensorium, drowsiness, nausea, vomiting, loss of appetite etc. Monitor his Blood Pressure and Blood Sugars strictly.Give him small frequent meals containing more of solids.Watch intake of total liquids.Salt and water restriction is required in case of edema.Hepatitis B vaccination should be also given. Hope it helps Dr. Kavita XXXXXXX
