Suggest Treatment For Suicidal Ideation With Depression

Question: i have constant homicidal and suicidal thoughts and fear of harming myself and others. i have been on all ssris with no relief. currentlly i am on topamax and risperadal for one month with no relief yet. i see a psyciastrist from u XXXXXXX and a psychologist. I have a constant thoughts of alligators and fear of using a gun and using a knife. Do you have any idea what this could be i have been not really diagnosed with any straightfforward diagnosis.

I am sorry to see that you have been experiencing such profound depression with suicidal and homicidal ideations. I see you have been to your medical provider and a psychologist to help with your ideations. Your therapists have you on topamax and risperdal in hopes of reducing your suicidal ideations. I also see that you have been in contact with our medical consultants and have been given information on treatment options.

I would suggest that you have depression with OCD. You have thoughts that become ingrained and recurrent that are interfering with your life. The medications should help and you may still need to have additional medications added to your treatment. I would also recommend that you do workbooks on OCD and depression. One book I recommend is the "Anxiety and Phobia Handbook" by Dr. Bourne. This is called cognitive behavioral therapy and may help with your distorted and ruminative thoughts.

why do i have thoughts about specific topics of allligators i never worked in a zoo and i dont have any pets. also with guns, i dont own a gun and have never used one but i have everyday thoughts of using one is thre a reason for this?

There must be some traumatic event in your childhood related to something horrible. You then are equating the alligator to that person or event. It is not that you want to kill alligators you are really trying to get rid of or destroy the event that caused such terrible psychological damage. The alligator represents a psychological trauma. Learning how to develop proper coping and dealing with the anger related to that trauma will help you lessen its impact in your life.

Inpatient can do nothing different for me i went before all they can do is adjust the medications and they dont work anyway do you think it could be anything like schizophrenia or bipolar?

I agree inpatient will not offer much except keep you from harming yourself or others. That is the real reason for inpatient care

One of the tools I recommend is using cognitive behavioral therapy. This is outlined in the book I mentioned "The Anxiety and Phobia workbook" by Dr. Bourne. This may allow you to control and defend yourself against these distorted thoughts. There are several other therapies including ACT that may also help. I recommend you discuss these options with your psychologist. Perhaps they can recommend a workbook that maybe better suited for you

i do response exposure therapy with my psychologist now and i dont benefit from it at all is cognitive behavioral therapy different then that?
