Brief Answer:
Clinical evaluation and investigations accordingly.

Detailed Answer:
Hi.
Thanks for your query.
Read your history and understood your concerns.
To recapitulate: Female/54 - pain in upper stomach on eating - 3 days - continues to lower - increases on walking - severe enough to feel that you will have to double over - runs for 2 days - continuing to lower back ...

Please give additional information for me to assist you better:
- Is there associated bloating, constipation or inability to pass stool and gases?
- Any loose stool or sticky (mucus), foul smelling?
- Any rumbling you can hear or feel inside of the abdomen?
- Any other information you would like to share...

The most probable causes are:
- Intestinal obstruction.
- Intestinal infection
- Infective focus in the abdomen that needs evaluation.

I would advise you the following:
- Go to ER ASAP.
- Urgent clinical evaluation, examination preferably by a General Surgeon.
X-ray of the abdomen in standing position for 2 reasons > to see for gas under diaphragm and gas fluid levels in the intestine along with other signs like dilated bowel loop and so on.
- Ultrasonography of the whole abdomen.
- Tests of blood, urine and stool.

These will give sufficient idea about the diagnosis and further tests like CT scan and so on are needed when needed.

I hope this answers helps you to get an early and proper diagnosis and a plan for proper management. Please feel free to ask for further relevant queries if you feel that there is a gap of communication.
