Suggest Treatment For Stiff Neck, Vomiting And Diarrhea

Question: Day 1-stiff neck, vomiting, 101 fever. Day 3-diarrhea with aches. Now day 7 with worse stiff neck/migraine. Blood work- elevated liver enz. Enlarged spleen.

Otherwise healthy 33 year old female. No alcohol use. Not overweight. Waiting on stool culture.

I am Dr.Ajay Panwar,a neurologist,here to answer your query.

Acute onset fever.headache and vomiting along with neck stiffness indicates meningeal/brain inflammation(possibly meningitis).Elevated liver enzymes and splenomegaly goes for some acute infectious etiology.It is hard to comment on the possibility of Migraine in presence of intracranial/meningeal inflammatory symptoms.
In this setting,I suggest that the patient must be immediately rushed to the emergency room for urgent evaluation.CSF analysis and Brain MRI/CT may have to be done.Early treatment will be the key to improvement.

Fever is gone now. Still have diarrhea, but vomiting is under control with zofran and prilosec. Migraine and neck pain is persistant. If fever is gone am I still at risk? Thanks.

Though fever is gone,but in view of neck stiffness,elevated liver enzymes and enlarged spleen,there are more than enough reasons to get examined by a physician.I maintain,that you should get yourself evaluated.This is something which I can't diagnose historically.It has to be investigated.
