Question: Hi my sister has these spots in her back? She had high cholesterol, high insulin, shes 16 years old. She also has problems with periods. Very irregular. Shes overweight

Thee are Keloids - raised, firm, excess scar that formed, perhaps at the site of previous acne lesions. Keloids are usually itchy. The tendency to develop keloids is genetic and they commonly develop on chest, shoulders and upper back.
Treatment of choice for a keloid is intralesional injection of triamcinolone acetonide depot steroid preparation. I suggest that you visit a dermatologist in your region for the needful.
