Suggest Treatment For Skin Infection In Thighs

Question: I have been suffering from skin infection in the thigh area for the last 6 months. my doc said it is furuncles and he prescribed T-bact Muphorine ointment, antibiotic and anti inflamation tablets. and use anti bactirial soap. once the course was done and after using the soap for a month, the infection is back now.. please help me tp get rid of this infection completely.

Hi, Dr. Kakkar,
i will be uploading the pic in a short while..
the infection is on my right inner thigh and outer thigh, there are lesions in two places.

couple of weeks back i under went Glucose test (fasting) and found out to be free of diabetes.
last time it happened in the third week of august.
have been taking

It starts like a small pimple and then grows in size. and very painful, once the head appears and drains with puss and then becomes normal duraton of 4 days to a week.

Recurrent furunculosis

Hi.

Recurrent furunculosis is common in those who are carriers of Staph aureus.

I would suggest a pus culture and sensitivity testing from the furuncle which would identify the antibiotic sensitivity of the bacterium.

Swabs should also be taken from the nose, throat, umbilicus, axillae and perineum to determine carrier state. Culture and sensitivities are required.

Nasal carriage is common in those who have recurrent furunculosis. This can be eradicated by application of topical 2% mupirocin ointment, twice daily, to the opening or just inside the nares for 5 days. This eradicates carrier state for 3 months. Re-colonisation is common

In chronic furunculosis the choice of oral antibiotic ideally should be guided by sensitivities. Empirical therapy with a penicillin group or macrolide group of drugs can be started till sensitivity report is awaited.

If I was the treating doctor I would treat you for two weeks initially guided by antibiotic sensitivity, but you may need a longer course of perhaps six or eight weeks.

General measures that might prove useful:

-Twice daily bathing with an antibacterial soap, preferably that which contains chlorhexidine would be useful.

-Wash sheets and underwear regularly in a hot wash (above 55°C). The clothes should be turned inside out.

-Keep surroundings clean

-Other members in the family should also be identified for carrier state and overt infections.

To summarise: If I was the treating doctor I would have asked for a pus culture sensitivity from the furuncle and all the possible carrier sites and guide my antibiotic therapy according to the report, may be for an extended time (2 weeks rather than for a week). Besides I would have asked you to apply 2% mupirocin ointment twice daily, just inside the nares, for 5 days, to eradicate nasal carriage.

I would suggest you to visit a specialist for culture sensitivity investigation as well as a prescription of these (topical as well as oral antibiotics)
