Suggest Treatment For Sinusitis And Rhinitis Medicamentosa

Question: Suffering sinusities since long time i.e. more than 2 years and can't breath unless put otrivin drops 2/3 times in a day. Though there was no nosal indischarge, but throat is irritating and thicky phlegm comes from throat. Viral attacks of cold & cough attacks frequently (every 3 months) and got fixed in 4-5 days, but sinus continus round the year. No other health problems except continus sinus and nose blocking. Prescribed Allegra M and used continusouly for 20 days, but no help. Generally treated with amoxocilin when serious cold & cough exist. Recently had CT-SCAN of PNS and send no need of surgery. It is said to be allergic. can you help me.

Thanks for the query.

I have read your query , reviewed your medications and understand your concern. Based on your description, I consider your clinical history as suggestive of Rhinitis medicamentosa. It occurs due to excess use of otrivin drops. Otrivin nose drops makes turbinates enlarge inside nose hence difficult to breath . It should not be used more than 7 days & now a days many patient comes with this situation due to self medication .

If I were your treating Doctor for this case , I would advice you
Saline nasal spray 4 puff each nostril 5 times daily like solspre ,
Combination Steroid nasal spray with antihistamine 2 puff each nostril twice daily like Duonase nasal spray ,
Tab Allegra M at night ,
Tab paracetamol twice daily ,
Antibiotic may be necessary if examination reveals features of infection.
Salt water gargle thrice daily .

You will get relief . Initial 5 days without Otrivin nose drops will be difficult since you become Addicted after that condition will improve day by day .

Thanks for the follow up query.

Use above complete treatment plan for 1 month except antibiotic & paracetamol for 7 days only .

Make sure Otrivin nose drops should not be used any further otherwise nasal turbinates enlargement will not revert back to normal .

Revert back to me after 1 month complete treatment plan .
