Suggest Treatment For Side Effects Of Sulpitac And Quetiapine

Question: Dear doctor, XXXXXXX here. I have changed the doctor as I was not getting respite. This doctor has prescribed me the following medication :
Sulpitac od 400 in the morning
Inderal la 40 in the morning
Procyclidine 2.5 mor/eve
Zapiz 0.25 in the night and half in the morning
Quitiapine SR 200 in the night
Sizopin half of 25 in the night
Amitryn 25 in the night
My symptoms are :
Anticipatory anxiety
Meeting phobia
IBS
Though my IBS is better but anticipatory anxiety still bothers me
I am having the following concerns with this medication:

Feel very drowsy and sedated in the morning
Speech problems like fumbling and tounue twisting while talking, people can't make it but I feel as I'll fumble and toungue will twist
Sleep has a lot of dreams
My walk has become slow and I feel slow overall
Wanted to check with you about sedation, walking slow and
feeling slow and speech problem
Are these the side effect of this medication, if yes, how do I sort them out. Do you think cutting down on some medicines will help, if yes then which one to cut down and how do I become totally free of anticipatory anxiety. I am also having blurred vision.

Yes these are definitely the side effects of medicines. You have been advised to take three different antipsychotic drugs, Sizopine, Sulpitac and Quetiapine. Out of these except Sizopine both other drugs are in high doses. The high doses of antipsychotics are the reason behind symptoms like speech problems, slowness, blurred vision, salivation etc. These are probably extrapyramidal effects of these medicines.

In my opinion either the doses should be adjusted again or some drug like Trihexyphenidyl should be added to your therapy. This will help to reduce the side effects and you will feel better.

If you want to cut down then Amisulpiride (Sulpitac) can be cut down to 100 mg and then to 50 mg. This will have good effects on mood and will reduce the EPS too.

The blurred vision is there due to side effects of too many antipsychotics in your prescription. High doses are resulting in anticholinergic side effects in form of blurred vision. If the symptoms don't improve with time then consult your doctor again for making dose adjustments.

Can procyclidine be the culprit as the net says it can cause blurred vision? I am on 3.75 in the morning and 2.5 in the night.

Hello thanks for asking again.

Yes the drug is too a culprit. The drug is given to prevent extrapyramidal side effects of antipsychotics. The anticholinergic actions result in blurred vision, constipation, urinary retention etc symptoms.

Start taking evening dose of 2.5 mg in afternoon and avoid taking in evening. If symptoms persist then reduce the 3.75 to 2.5. This should help you.
