Question: after all these tests,what can be the problem. Breathing has not gotten any better !

Hello dear, thanks for your question on HCM.
I can understand your situation and problem.
Your extensive work up for heart disease,pulmonary disease like bronchitis and gastric diseases are normal,but still you have breathing difficulty and loss of voice.
So in my opinion you are having most probably obstructive pathology of upper airway. In this condition both of your symptoms are seen.
Another rare possibility is pulmonary embolism (PE).
So need to know few things first.
1. Your height,weight (KGS).
2. Do you smoke?
3. Have you under gone CT pulmonary angiography to rule out pulmonary embolism (PE).
4. Do you have any problem in sleep? Like snoring or choking attacks in night or sudden awakening in the night or excessive day time sleepiness.
So please reply me answers of these questions,so that I can guide you better.
Your all routine tests are normal, so we need to think out if the box to diagnose your problem.

I have not had a Pulmonary Angiography that I am aware of.
Sleep well at night but still sleepy during the day. Sleeping approximately 12-14 hours a day.

Hello dear, thanks for your follow up question.
In my opinion you should get sleep study because day time sleepiness can be seen in obstructive sleep apnoea (OSA). And it is related to upper airway obstruction and your symptoms.
So sleep study is needed to rule out OSA.

As all your routine investigations are normal. So we need to search for uncommon causes.
So I suggest sleep study.

I also advice you to get CT pulmonary angiography to rule out pulmonary embolism(PE).
As PE is not that uncommon if common causes of breathlessness are ruled out.
